First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be started the same day as the consultation, schedule permitting. However, a complex medical history or treatment plan may require we schedule an appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any X-rays* if applicable
- A list of medications you are presently taking (including vitamins and supplements)
- If you have dental insurance, bring your insurance card or information.
MINOR PATIENTS: A parent or guardian must accompany all patients under 18 at the consultation visit, and any subsequent appointments. The parent or guardian accompanying the minor is the person responsible for any payment due at each appointment. We will not attempt to collect payment from a parent or guardian not present at that appointment.
MEDICAL CONCERNS: Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med.)
Digital X-Rays
Digital X-rays are far superior to traditional radiography. Our digital system emits far less radiation and doesn’t produce paper/film waste. The images appear on a computer screen for us to view and store in your electronic records. This is better for you and the environment!
*If your dentist has taken recent x-rays (within 6-weeks), you may request that they forward them to our office. We will take new images at your appointment. It can be helpful to have the additional images for comparison, however we cannot diagnose from an x-ray not taken by our office.
